Under the Kaiser HMO plan, you are required to receive all of your care through an integrated system of participating physicians, hospitals and other health care providers. In other words, you must use Kaiser doctors, hospitals and pharmacies. Most services are covered at 100% after you pay a relatively small copayment. To participate in the Kaiser Permanente HMO plan, you must live in one of the zip code service areas served by this HMO. Refer to the Evidence of Coverage for a detailed list of this plan's covered services, exclusions and limitations.
