No appointment is necessary to meet with a Benefits Analyst at the HSS office. Members are served in the order of their arrival.

Municipal Executives EBS Enrollment

Eligible participants in the Municipal Executives (MEA) plan have the option of allocating flexible credits and enrolling (or re-enrolling) in an FSA by telephone/fax. A limited number of in-person EBS appointments are also available at the HSS office. Call EBS at (888) 327-2770 for more information. See MEA flexible credit options.

Municipal Attorneys Cash Back

Eligible participants in the Municipal Attorneys Association who are enrolled in an HSS health plan have a cashback election option. If you are not making changes to your Cash Back election, you do not need to return this form - your Cash Back election will remain the same for plan year 2010-2011. If you would like to make changes effective July 1, 2010, submit this MAA Cashback Election Form to HSS by 5:30pm, April 30, 2010.

 

Change Your Benefit Elections

Open Enrollment is your annual opportunity to make changes to your HSS administered health benefit elections without any qualifying event requirements. This means you may elect a different medical and/or dental plan. You may also add or drop dependents from your coverage. And active City of San Francisco employees have the option of enrolling (or re-enrolling) in Healthcare and/or Dependent Care Flexible Spending Accounts (FSAs). Any changes you make to your health benefits during Open Enrollment will take effect July 1, 2010.

Application Deadline: April 30, 2010

To make changes to your health benefit choices HSS must receive your completed Open Enrollment application and required eligibility documentaiion no later than 5:30pm on April 30. (See the Open Enrollment application form for a checklist of required eligibility documentation.) You may mail, fax or drop off your application to HSS. If you choose to fax, please do not fax the same application multiple times. Provide your email address on the application and HSS will confirm receipt of your fax via email within two business days.

Ineligible Dependent Amnesty

If any of the dependents currently covered on your plan are ineligible for Health Service System coverage you will be responsible for the costs of all premiums and benefits provided dating back to the original date of ineligibility. Dependent audits will be taking place later this year. Drop ineligible dependents from your coverage during April 2010 Open Enrollment and HSS will give you amnesty from penalties.
